    A woman suffering from a painful complication of breast enhancement surgery reaches out to The Doctors for help. Find out what went wrong — and if it can be fixed.

    After giving birth to twin boys, Darlene felt self-conscious about her sagging, unsymmetrical breasts, so she decided to have a breast augmentation.

    “I just wanted to do something to feel good about my body,” she says.

    She says at first she was happy with the results and felt more confident.

    Then, about six years after the surgery, that changed. Darlene was watching TV and felt a cool sensation as if water was leaking in her left breast.

    “As I was sitting there, I could actually see it deflating, getting smaller, kind of like a balloon,” she recalls.

    A week later, Darlene began to feel an intense pinching feeling that wouldn’t go away, so she called the surgeon who performed her breast augmentation. He told her the only way to relieve the discomfort was to take the implant out, but she couldn’t afford the surgery.

    “I have been living in pain now for two-and-a-half years,” Darlene says. “It’s with me all the time, 24/7. I can never get away from it.”

    The pain is so bad that it interferes with Darlene’s daily life, and she even has considered ways she could take out the implant herself, using a razor blade.

    “I just realized how desperate I really was,” she says.

    The Doctors send Darlene to plastic surgeon Dr. Gary Motykie for a consultation. Dr. Motykie explains that as the implant deflates, the pocket starts to implode, like a vacuum, and you get knuckles on the sides of the implant that can pinch the surrounding tissue. As the tissue collapses, it can pull on nerves and scar tissue, causing discomfort. After examining Darlene, Dr. Motykie finds that her right breast also likely has scar tissue, and the implant is also at risk of rupturing.

    A few days after the consultation, Dr. Motykie explains that an implant deflation doesn’t typically cause pain.

    “It’s called a silent leak, in most cases. Even in the case of saline, it may deflate, but it doesn’t hurt.” he says. “So, in her case, the pain may be more related to the capsule or the scar tissue contracting causing the pain but not actually from the deflation itself.”

    Dr. Motykie recommends that both implants be removed and replaced with new, smooth implants, and he offers to perform the surgery free of charge.

    “I think she’ll get relief, not just physically, but emotionally, mentally, everything,” he says.

    Breast implant surgery is the second-most popular cosmetic surgery procedure in the US today after liposuction. Breast implants qualities have improved dramatically over the years following the FDA approvals, surgical techniques have become advanced, and overall success rates are very high.

    However, a breast augmentation patient will typically need to replace the implants once or more during her lifetime. The implant may get damaged or develop other complications due to a variety of reasons, which makes it necessary to remove or replace it with breast revision surgery.

    Case of Darlene, Pleasanton, CA

    Darlene, 47, had received saline breast implants nine years ago, when she suffered an implant rupture in 2012. She was in extreme pain, and rushed to her plastic surgeon who refused to treat her unless she agreed to pay for the breast revision procedure.     Why do Breast Implant Warranties Matter?

    Breast implants are available in two primary types: saline and silicone gel implants. The outer shell of the implants in both cases is made of a firm silicone material. One of the challenges with breast implants is that one or both implants may get damaged or ruptured at some point, or develop a condition called capsular contracture at some point of time in the future.

    The precise causes for this condition remain unknown, but it usually requires implant replacement. In some cases, the implant may become misaligned or mal-positioned, if not damaged. The only way to correct this condition is to have the breast implants removed or replaced with new implants, which involves breast revisions surgery. Sometimes the patient may require a voluntary replacement of implants due to a change in aesthetic preferences.

    With an adequate implant warranty, the patient is secure about the future option of getting breast implants replaced without having to pay for them again. Most implant manufacturers offer warranties with minor variations in their terms and conditions.

    Mentor has recently updated their breast implant warranties, which patients should know about. The old warranty covered only a rupture with lifetime replacement and 10 years of $3,500 financial assistance. Now Mentor has added a capsular contracture warranty to all gel implants (round, shaped, smooth, and textured) for capsular contracture, late Seroma or double capsule.

    3-year warranty for capsular contracture (Bake Grade III/IV), late seroma and double capsule: Mentor will provide two free replacement implants for these patients (the affected side and the contralateral side).

    Enhanced Warranty: $200 Charge to Patient (payment within 45 days of surgery)

    10 year warranty for capsular contracture (Bake Grade III/IV), late seroma and double capsule: Mentor will provide two free implants for these patients in addition to up to $3,500 of financial assistance.

    The ‘thut’ is a unique term coined to describe the loss of definition in the hip area. Many experts blame the sedentary lifestyles, which contribute to a growing number of people losing the definition between their thighs and buttocks. In specific terms, the ‘thut’ is said to occur when the muscles on the back of a woman’s legs are under-developed, which results in the buttock and thigh to appear a single anatomical unit.

    As an experienced plastic surgeon, Dr. Gary Motykie tells patients that it is possible to improve this condition with appropriately monitored fitness training and exercise programs. Muscular origins of the thut are a part of the postural support group that helps determine walking, standing and lifting movements. The laxity of muscles in this group, or lack of muscular development may be attributed in large part to a sedentary lifestyle.

    The gradual shifts in working culture over the years have led to a situation where a majority of working people perform their jobs while seated in a desk chair for most part of the day rather than working in physically laborious occupations. To counter this kind of work life, it is important for a person to exercise regularly. Muscle laxity can cause a situation where the butt appears to drop down into the leg, and causes a loss of lift.

    Not a Genetic Condition

    Dr. Motykie says that the thut is not hereditary. It is essentially a product of definition and muscle tone. If the problem was genetically linked, it may have been much more difficult to correct the shape of muscles. However, the thut is largely a problem of muscle tone and active use. Therefore, the problem is entirely treatable if the individual focuses on targeted and appropriate physical exercises and activities.

    The thut has been widely recognized among fitness experts over the last few years. Especially since 2011, the condition has been commonly discussed among fitness enthusiasts and experts alike. The first mention of the word ‘thut’ in the Urban Dictionary came way back in 2007. However, many people are still unclear about what the term means, and how the condition can be corrected.
